Role Overview
As our Community-Based Palliative Care Nurse Practitioner, you bring advanced clinical expertise and human connection to individuals and families living with serious illness. In collaboration with the medical director and interdisciplinary team, you’ll provide patient-centered, holistic care that focuses on quality of life, symptom management, and aligning treatment with personal goals.
You’ll serve people in their homes, assisted living, or wherever they live; offering comfort, clarity, and continuity in the middle spaces of serious illness.

Key Responsibilities
  • Provide direct medical management, prescribe treatments, and alleviate suffering through expert symptom control.
  • Conduct full assessments (history, physicals, diagnostics) and develop individualized care plans anchored in patient and family goals.
  • Lead advance care planning discussions that help patients define values and preferences.
  • Perform regulatory face-to-face visits when required for hospice or palliative eligibility, ensuring compliance with CMS guidelines.
  • Collaborate closely with the interdisciplinary team (nursing, social work, spiritual care, volunteers) and outside providers to ensure seamless care.
  • Support patients and caregivers through teaching, counseling, and integrating resources that reduce caregiver strain.
  • Maintain accurate, timely documentation in the medical record and contribute to team communication.
  • Participate in quality improvement projects, drawing on CAPC tools and best practices to strengthen service delivery.
  • Build and nurture community relationships, strengthening awareness of palliative care options.

Qualifications
  • Graduate of an accredited Nurse Practitioner program (Master’s or higher).
  • Current Montana NP license.
  • At least 1 year of NP practice experience, plus 1 year of hospice or palliative care experience preferred.
  • National certification (e.g., ACHPN, CHPN, geriatrics) strongly preferred or willingness to pursue.
  • Strong assessment and communication skills, with confidence in goals-of-care conversations.
  • Licensed driver with insured, reliable vehicle.
  • Physical ability to perform home visits (walking, lifting, bending).

Additional Expectations
  • Full-time (25–40 hours/week) with some flexibility.
  • In-person, community-based role serving patients where they live.
  • Occasional after-hours availability for urgent needs.
  • Commitment to ongoing professional development and use of CAPC resources.
